Our mission is to educate parents, youth, health care professionals, and all other community members about the truth behind tobacco use and the effects of nicotine addiction on health disparities. Despite significant progress, tobacco/nicotine use remains the leading preventable cause of death and disease in the US. We provide evidence-based, statewide tobacco/nicotine control programs that are comprehensive, sustained, and accountable to reduce tobacco-related diseases and deaths.

QUITLINE PATIENT REFERRAL ONLINE FORM
As a healthcare provider, you are in a perfect position to be the catalyst in inspiring your patients to live healthier, tobacco/nicotine-free lives. Patients look to you for help and advice when it comes to their overall health.
