The Southwest Utah Public Health Department (SWUPHD) respects the privacy of every individual who visits our website. This Privacy Statement outlines the information SWUPHD will collect and how that information will be used. We are committed to protecting the privacy and accuracy of confidential information to the extent possible, subject to provisions of state and federal law. We do not re-distribute or sell personal information collected on our website.

PERSONALLY IDENTIFIABLE INFORMATION

When visiting SWUPHD website (swuhealth.org) we don’t collect any personally identifiable information about you (such as name, address, telephone number, social, or email address) unless you provide it to us voluntarily (e.g., completing a webform).

These forms require users to give contact information (e.g., for Tobacco Specialty Licensing purposes). Once collected, the information is stored electronically indefinitely. Should you wish not to provide any Personally Identifiable Information, you may utilize the services by using the appropriate non electronic means.

USE OF PERSONALLY IDENTIFIABLE INFORMATION

SWUPHD will only use personal information collected from this site for the purpose of communication back to individuals who contact us via the site. SWUPHD uses browser-IP-address information and anonymous-browser history to report information about site access and for profiling purposes. This information is used to improve web presentation and utilization. IP address information may be used for troubleshooting purposes. All of these matters and other information may be lawfully classified as Private or Controlled in accordance with the Government Records Access and Management Act (GRAMA). SWUPHD may store and process your personally identifiable information to better understand your needs and how we may improve our electronic services and we may use your personally identifiable information to contact you if so, requested by you (for example, we may send you a questionnaire or we may respond to your comments or requests for information).

SWUPHD will not give your personally identifiable information to any outside organization unless required by law.

NON-DISCRIMINATION AND EQUAL ACCESS POLICY

In accordance with Federal civil rights law and U.S. Department of Agriculture (USDA) civil rights regulations and policies, the USDA, its Agencies, offices, and employees, and institutions participating in or administering USDA programs are prohibited from discriminating based on race, color, national origin, sex, disability, age, or reprisal or retaliation for prior civil rights activity in any program or activity conducted or funded by USDA. To file a program complaint of discrimination, complete the USDA Program Discrimination Complaint Form, (AD-3027) found online at: http://www.ocio.usda.gov/sites/default/files/docs/2012/Complain_combined_6_8_12.pdf, and at any USDA office, or write a letter addressed to USDA and provide in the letter all of the information requested in the form.
