Any business preparing and serving food in the five counties served by the Southwest Utah Public Health Department is required to maintain an annual permit along with a trained Certified Food Manager (CFM) who oversees safe food handling.
The food service businesses listed here are in critical violation by operating without a permit and/or CFM (indicated by an asterisk*). All have received multiple notices from the health department but have not responded. The owners/managers have been notified that their business will be listed here if response to a final notice is not received in a timely manner. Refusal to comply may result in closure.
