Our clinics focus on vaccine-preventable diseases for children and adults including Diphtheria, Pertussis (Whooping Cough), Tetanus (Lock Jaw), Polio, Measles, Mumps, Rubella, Haemophilus Influenza, Hepatitis A, Hepatitis B, Typhoid, Chicken Pox, Meningococcal, Child & Adult Pneumonia, and Flu.

Low or no cost immunizations for those who qualify. We bill most major insurance companies. See list of accepted insurances HERE.
You can now access a copy of immunization records by visiting the Utah Statewide Immunization Information System.

Immunization Exemptions: 

Starting July 1, 2018, individuals wishing to receive an exemption waiver for religious, medical, or personal reasons can complete the immunization education module by visiting their local health department office or visiting

Once on the website, click on the “Online Immunization Education Module” tab to access the module.
