Printed vaccination cards only (photo copy not accepted). Parents or legal guardians must be present for child to be vaccinated. Practice social distancing or use mask when distancing isn’t possible. Stay at home if you are sick. If you are sick with Covid-19 or tested positive, wait until after the recommended isolation period to get vaccinated.
