Health Equity

Our community outreach team is focused on helping achieve health equity in our community. What is health equity? This is when every person has access to resources that allow them to reach their full health potential and eliminate health disparities caused by varying life circumstances. Often it can be difficult to find and access resources that can help us achieve health equity which is why we have community health workers that can help connect you to resources and partners in the community that will achieve this goal.

What is a community health worker?

Our community health workers are here for you! A community health worker’s goal is to find and support those that do not have full access to mental and physical medical care as well as many other assistance programs that community partners can help provide. They are here to help connect the community to important and necessary resources that can help in times of need.
