How to Apply:

  1. Fill out application at your local health department. You can print out the copy of the application form (see below) and fill it out ahead of time.
  2. Show a current state issued picture ID.
  3. The fee is $16 for the first document and $8 for each copy for each individual.
  4. It can be processed while you wait.
  5. Records are available back to 1977. Older records must be requested from the Office of Vital Records and Statistics, 288 N 1460 West, P.O. Box 141012, Salt Lake City, UT 84114-1012. The time frame is 3 to 4 weeks. These certificates can be expedited in a 24 hour time period for a fee of $30.
  6. For genealogy purposes, contact your local office for further information.

Utah law allows families to care for their dead without the services of a licensed funeral director. Families that do not retain a funeral director must file a death certificate and comply with state laws and rules regarding the disposition of human remains
