How to Apply

1. Fill out application at your local health department. You can print out the copy of the application form available on this web site and fill it out ahead of time.
2. Show a current state issued picture ID.
3. The cost is $18 for the first document and $8 for each copy for that one.
4. Some older records are not immediately available. For the $18 fee, the application must be sent in to the Office of Vital Records and Statistics, 288N 1460 West, P.O. Box 141012, Salt Lake City, UT, 84114-1012. The time frame is 3 to 4 weeks. For faster service, the fee is $30, and the record is available in 24 hours.
